Building a form

Design the questions a new patient answers before their first visit.

Creating a form

Go to Forms in the sidebar and choose New form. The builder lets you add, reorder, and remove questions — each question uses the same field types available for patient custom fields (short text, paragraph, email, yes/no, single or multiple select, date, number, percentage), so answers map cleanly onto a patient record later.

Editing a form

Reopen any form from the Forms list to change its questions. Editing a form doesn't change answers already submitted against the previous version — those stay exactly as the patient answered them.

Tip

Keep a form short. It's presented to the patient as one screen at a time (see Sharing the public link), so a long form means a lot of screens to tap through before their first visit.

A form can also include a terms & conditions block and require a signature before it can be submitted — see Signatures & consent.

Grouping a few questions onto one screen

Most questions get their own screen, but a few fields that read as one answer — First name / Last name, City / State / ZIP — feel better shown together. Add the questions you want grouped, then click Split into its own screen on a question to toggle it, or use + Add name fields (First + Last) as a shortcut for the common case. The builder shows each screen as its own box, labeled "Screen 1", "Screen 2", and so on — drag questions between screens by moving them up/down until they land next to the ones they should share a screen with, then use the merge/split link to group or ungroup them.

Deleting a form

Deleting a form moves it to the trash. Existing submissions already collected against it are unaffected. See Trash & recovery to restore a deleted form.
