Sharing the public link

What a patient sees when they open your form link, and how their answers are saved.

Every form has its own public URL (/forms/[form id]) that needs no login — send it to a patient by text, email, or however you'd normally reach them before their visit.

What the patient sees

The public form opens full-screen, with no app navigation around it, and presents a conversational, one-question-at-a-time flow. As the patient answers, their progress is saved to their device automatically, so closing the tab partway through and reopening the link later picks up where they left off.

Note

If a patient completes a form and reopens the same link on the same device afterward, Goji detects the earlier submission and won't let them submit a duplicate.

If the form requires a signature, the last step asks the patient to type their name and sign before the Submit button becomes Continue to sign — see Signatures & consent.

After submission

A completed form doesn't create a patient by itself — it lands in your organization's Form submissions inbox, where you decide whether to create a new patient from it, attach it to an existing one, or dismiss it.
