A patient moves to Portland. Another finishes their course of treatment, and a third you decided not to keep seeing. None of them has been in for months, and on the roster all three read as Lapsed, which is the list you use to decide who to reach out to.
Recording it
Open the patient's overview and choose Record care ended. Any member of your practice can do it. The form asks for three things.
- Reason: Moved away, Completed care, Transferred to another provider, Deceased, Practice ended care or Other. Completed care is the default.
- Care ended on: a date, filled in as today. Backdate it to when care actually ended; a date in the future is refused.
- Note: optional free text, up to 2,000 characters.
Once saved, the form is replaced by a Care ended line showing the reason, the date and your note. The patient's Status on the roster becomes Care ended, and that wins over every other status, so they leave Lapsed, Overdue and the rest. They appear in the Care ended view, and you can sort and filter on the status like any other. Care status explains the full order.
When it lifts
Care ended is a record of what you decided, not a lock. If the patient is seen again, it stops applying by itself and their Status goes back to being worked out from visits and cadence. A visit counts when it is attended on a day after the one you recorded it; a visit on the same day does not lift it, and neither does an appointment that is only booked. Imported attended visits count too.
A patient who comes back this way gets the small Back tag on the roster for 60 days after that visit, a prompt to welcome them. The entry stays in the record, and a later one supersedes it.
Deceased is the exception. It never lifts, and appointment emails and texts are not sent to a patient recorded as deceased.
Recorded by mistake
Choose Remove (recorded by mistake) and the entry stops applying straight away. It is kept for the record rather than erased. Creating, viewing and removing an entry are each written to the audit log by id only; the reason and note are not.
Closing out a whole roster
To record care ended for many patients at once, for example after moving over from another system, see import former patients. Those entries are ordinary ones and lift the same way.
