Asking for a first name on one screen and a last name on the next feels like a form talking to itself. Some questions belong together.
Group questions in the builder
In the form editor, every screen is its own box labeled Screen 1, Screen 2 and so on. Three controls decide what shares a box:
- + Add question to this screen adds a new question to the end of that box.
- Merge into screen 1 (the number is the screen before) sits in the header of every screen after the first and folds the whole screen into the one above it.
- Split into its own screen is in the ⋮ menu of a grouped question and gives it a screen of its own again.
The up and down arrows beside a question move it, so you can bring a question next to the ones it should share a screen with and then merge. For the common case, + Add name fields (First + Last) adds a First name and a Last name question already grouped.
What the patient sees
A grouped screen shows all its questions together, and the counter and progress bar count screens, not questions. OK checks every question on the screen, so a missing required answer stops the patient there with the message under that question. The Press Enter to continue hint is left off grouped screens, because Enter inside one of several fields doesn't mean the screen is done.
A form's own page shows how many questions and screens it has. If you switch the form to one page, grouping stops mattering: every question shows on the same page.
