A new patient finishes your intake on their phone in the waiting room, or the night before. The next thing they see should tell them where to park, not just that something was received.
Where to set it
Open the form’s edit page and find the Ending card, described as shown after a successful submission. It has a Headline, a Message, and an optional Button label and Button link. Changes save when you click out of a field.
What patients see
- Leave the headline blank and the screen says Thank you! Leave the message blank and it says Your answers have been submitted.
- The message takes light formatting: bold, italic, links in label form, bulleted and numbered lists, and short headings.
- The button only appears when it has a label and a usable link. A link can start with https://, http:// or mailto:, or be a path on this site such as /. Anything else is ignored and no button shows.
- Below your message, patients still get a Submit another response button.
If the form also asks for a signature, the signature comes first and this screen follows it.
