A consent form is only useful if you can later show what the patient was looking at when they agreed. Goji lets you put your own consent wording in front of the patient, make them reach the end of it, and finish with a signature, all inside the same phone-friendly flow as the rest of the form.
Consent text patients scroll through
In the form builder, choose + Add terms & conditions block. You give it a Section heading and paste the full text of your treatment consent, privacy notice or anything else you want read. A form can have as many as it needs, anywhere in the question order.
The patient sees the whole text inline. The I have read and agree to the above checkbox stays disabled until they have scrolled to the end, and the screen says Scroll to the end to continue. If the text is short enough to read without scrolling, the checkbox is available straight away. A new terms block is Required by default, so the patient can't move past it without agreeing. Several of our free templates already include consent wording you can edit.
Require a signature to submit
On the form's edit page, tick Require a signature to submit this form and write the Signature attestation, the line shown above the signature pad. If you leave it blank, patients see: By signing below, I attest that the information provided above is accurate to the best of my knowledge.
With this on, answering the last question no longer submits the form. The button reads Continue to sign, and the patient lands on a final Sign to submit step. They type their full name, draw their signature on the pad with a finger or mouse, and choose Submit. Clear on the pad lets them start over. If either is missing they see Please type your name and sign above before submitting. The same check runs on the server, so a submission that skips it is refused with A signature is required to submit this form.
There is one signature per submission, not one per question, like the single signature at the end of a paper packet. It works the same with grouped screens and one-page forms.
What Goji keeps, and where you see it
- The typed name and the drawn signature, saved as an image.
- The exact attestation and the text of every terms block on the form at that moment.
- The time of signing, and the IP address and browser of the person who submitted.
The signed submission arrives in your submissions inbox. Expand View answers and the card shows Signed by the name, the date, the IP address and the signature image. Download PDF adds the attestation, the exact time and the browser, and shows each terms block as Agreed or Not agreed.
