Restyle a form live

Try colors, corners and type on a real sample intake form on the forms page, and see the embed code they would give you. No account needed.

Tactile editorial artwork for Forms Playground

A form on your own website has to look like your website, and a color name in a settings list does not tell you whether terracotta on oat will read well. The sample on the forms page lets you find out before you have a form or an account.

Where to find it and what you can change

The sample sits beside the introduction on Intake forms, in a frame labeled yourpractice.com/new-patients. Press Customize this form and a window titled Make the form yours opens with the controls next to a larger preview.

Change a choice and the form beside it changes.
  • Look: Sage, Terracotta, Ocean, Blossom or Ink, or Custom, which gives you a color picker for Buttons & highlights and one for Background.
  • Corners: Square, Soft or Round.
  • Type: Clean, Serif, Rounded or Lora.
  • Form: switch between the public intake templates, so you can see a short form and a long one in the same style.

Text on top of your accent color switches between white and dark on its own, so a pale or very dark choice stays readable.

What is real, and what is kept

The form in the frame is the real patient-facing form, the one a template page previews, not a picture of one. You can answer its questions and step through it. Submitting goes nowhere, and the form never writes a draft or a finished marker to your device, so you can run it as often as you like and nothing you type is kept.

Under the controls, the line headed What you'd paste on your site shows the tag with the attributes your choices would produce, the same ones the embed guide writes. Lora is a Google Font, so choosing it makes your browser fetch that font from Google. Clean, Serif and Rounded use fonts already on your device.

From the sample to your site

Once you have an account, open a form's menu and choose Get this on your website. The guide has the same looks plus your own questions in the preview, and Embed forms on your site covers the three steps, the options you can write by hand and the events your page can listen for. Starting from a template gets you a form to restyle quickly.

Try it in your own practice.
