Free form templates

Start from eight ready-made intake, consent, privacy-notice and cancellation forms, then edit your own copy.

Tactile editorial artwork for Forms Template Library

A blank form is the hardest one to write, and a long paper intake is a lot to retype. The template library gives you a finished form to cut down or build up, written for a treatment room rather than a generic clinic.

Search, filter by type, and use one.

What is in the library

There are eight templates in three groups:

  • Intake forms: New Patient Intake (Quick), Pediatric Intake, Ages 0–12 (Quick), New Patient Intake (Comprehensive) and Pediatric Health History, Ages 0–12 (Comprehensive). The quick ones ask the handful of things a first visit needs. The comprehensive ones are long, faithful conversions of full paper forms, so ask patients to complete them before the visit, not in the waiting room.
  • Consent forms: Informed Consent for Acupuncture, Herbal Medicine Consent and HIPAA Notice of Privacy Practices Acknowledgement.
  • Policies: Cancellation & No-Show Policy.

The consent and policy templates already end with a signing step, with the wording the patient agreed to saved alongside the signature. Several use a section the patient has to read before agreeing.

Use one, then make it yours

Open Forms in the sidebar. Start from a template is open on a brand-new practice and collapsed once you have forms of your own. Search by name or description, narrow by Intake forms, Consent forms or Policies, and choose Use this template. Goji creates your own copy and opens it in the form editor, where you reword, reorder, add or remove questions like any other form (how the builder works). Editing your copy never changes the template or anyone else's copy.

Then share it as a public link or embed it on your site. If you would rather start from a form you already have, paper intake conversion turns a PDF into one.

Preview before you sign up

The intake templates also have public pages under Form templates, each with a live preview of the form your patients would see, a list of every question, and notes on what to adjust. Nothing you enter in the preview is saved, and you do not need an account. The consent and policy templates are offered only inside the app for now. You can also try the intake templates in the live restyling sample on the forms page.

Check the wording before patients see it

The templates are general starting points. Every one carries a note saying so, and the consent and policy ones, plus a Legal template tag on each card, ask you to have them reviewed for your location before use. Some details are yours to fill in: the Cancellation & No-Show Policy has a 24-hour window and a late-cancellation fee as placeholders, and the HIPAA acknowledgement records that a patient received your Notice of Privacy Practices but does not replace writing or handing out that notice. Herb rules differ by state, so check yours.

Try it in your own practice.
