Intake forms, no account needed.

The patient who won't sign up for an account just to fill out a form isn't unusual — that's most of them, and every account you require is a form you don't get back. Goji sends a plain link: they tap it, answer, and they're done. No password, no app, nothing to download. Answers arrive as structured data ready to review, not a stack of paper waiting to be retyped after the visit.

No portal required

Patient intake forms with no portal login.

Text it, email it, or hand it out however you'd normally reach a patient before their visit. The form opens full-screen with no app navigation around it, one question at a time. Progress saves to their device automatically, so closing the tab partway through and reopening the link later picks up right where they left off — and Goji won't let the same link submit twice from the same device.

Signatures & consent

Electronic signatures and consent that hold up — ESIGN Act and UETA.

Add a terms & conditions block the patient has to actually scroll through before agreeing, and turn on a required signature to collect a typed name and drawn signature before submission. Every signature is stored with the exact wording it was signed against, a timestamp, and the submitter's IP and browser — enough to satisfy ESIGN Act/UETA requirements for a valid electronic signature.

After submission

From form submission to patient record, without re-typing it.

A submission doesn't create a patient by itself — it lands in a Form submissions inbox, where staff choose to create a new patient from it, attach it to an existing one, or dismiss it. Custom fields map onto the same fields your patient and session records already use, so answers don't need re-entry. Nothing about a submitted form is stuck: a duplicate submission or a form from someone who never books is dismissed in a click, not carried around as a half-patient.

A portal, if you ever want one

An optional patient portal — never required to complete a form.

Some practices like giving patients an account to see their own upcoming visits — Goji supports that too. But it's a separate, optional feature. A patient can fill out every intake form you send without ever creating one.
