Forms in the portal

Forms a patient still owes show on their portal calendar, and finishing one returns them to the portal with the answers already matched to them.

A form should be one tap from the visit it is for.

Outstanding forms, right on the calendar.

What shows up, and when

The panel lists forms that the services on a patient's upcoming confirmed visits require. You attach a form to a service in Scheduling settings; an attachment made there is required and asked once. Forms marked optional or every-visit are labelled as such where they appear. Each row shows the form name and the date of the visit it is for.

  • A once-ever form drops off the list as soon as it has been submitted for any of that patient's visits, so a patient with two upcoming visits is not asked twice.
  • An every-visit form is asked again for each upcoming visit and clears per visit.
  • If one sign-in is linked to more than one patient or practice, the panel names the patient and practice on each row. The Appointments item in the portal's side navigation also shows how many forms are waiting.

When the patient finishes

Opening a form takes the patient to the usual form page with the visit attached. When the sign-in is linked to that visit's patient, the submission is recorded against that patient and marked converted straight away. It skips the pending queue in Form submissions, where only unmatched ones wait for you. The patient is sent back to their appointments page instead of seeing a generic thank-you screen.

Without a portal

The portal is optional. Without one, the same form opens from a plain public link, and a patient who is not signed in sees the end screen you set on the form. If the visit is known from a booking link, the submission is pre-linked to that patient but still waits for you to accept it, since the person who booked may not be the person who filled it in. From a patient's own page, staff can see which required forms are still missing, whether or not the patient has portal access. See Invitations for giving a patient access and the portal calendar for the rest of what they see.

Try it in your own practice.
