You spend part of a visit on a real evaluation and part on needling. Without modifier 25, many insurers treat the consultation as part of the acupuncture and deny it as bundled.
What gets suggested
On the superbill page, a patient's first chart gets 99203 offered and every chart after it gets 99213, each with an Add button. If a herbal prescription was recorded for the visit, 99214 is offered too. Only codes your practice has turned on under Billing codes are suggested, and nothing is ticked for you.
When modifier 25 prints
When you add an E/M code to a visit that also has a needling line, the superbill prints it as 99213-25. The modifier is attached automatically; you don't type it. If the visit has no acupuncture line, for example a service set up with acupuncture codes off for an herbal consult, or a visit under 8 minutes of treatment, the consultation prints plain. Modifier 25 applies to the consultation codes only, not to preventive visits or herbs.
Before you add one
Each consultation code carries a note to choose the level by medical decision-making or total time, and that some payers don't cover a same-day consultation at all, so check yours. If the patient's last full consultation was fewer than 30 days ago, the suggestion shows a warning that a full E/M isn't meant for every visit. It is a warning only, and you can still add the code.
The patient's superbill still goes to their own insurer, not through Goji. See Billing codes for turning consultations on and Suggested CPT codes for how the needling lines are worked out.
