A patient asks for the intake link, a colleague wants the form on your website, and you want to see what it looks like. All of that starts from the same place, the ⋮ menu.
What each item does
- Get this on your website opens the embed wizard: pick a look, then copy a snippet to paste into your site. See Embed a form.
- Share link copies the form's public address to your clipboard, and the item reads Link copied! for a couple of seconds.
- Open form opens the real patient-facing form in a new tab.
- Edit form goes to the editor, and Delete sits below a divider at the end.
Delete asks you to confirm first. It moves the form to the trash rather than erasing it: submissions already collected are kept either way, and the form can be restored from Trash.
Where the menu appears
It is on every row of the Forms list with all five items. On a form's own page it has Get this on your website, Share link, Open form and Delete, since Edit form is already a button beside it. On the editor it has the first three. A Share this form card on the form's page repeats the link, Share link, Open form and Get this on your website as plain buttons.
What a form's page shows
Click a form in the list to see its page: how many questions and screens it has, the number of submissions and how many are waiting for review, and whether a signature is required. Below that, a Preview the form disclosure shows the real form (nothing entered in the preview is saved or sent), a Questions list grouped by screen, and a Sent automatically with list of the services that ask patients to complete it, marked Required or Optional, with every visit where that applies. Recent submissions shows date and status (Pending, Converted or Dismissed) only, with View all in inbox taking you to the forms inbox filtered to that form.
