A new patient fills in your intake on their phone the night before, and so does someone who already has a chart under a slightly different spelling. If every submission quietly made a patient, you would find the duplicates weeks later. Goji holds each completed form for a look first.
Where submissions arrive
Forms in the sidebar opens on the Submissions tab; the Forms tab beside it is the form builder. The sidebar shows how many submissions are still pending. It opens on Pending, and the Converted, Handled, Dismissed and All tabs show the rest. A form's details page has a View all in inbox link that shows only that form's submissions.
Every card has a View answers toggle that shows the questions and answers as the patient filled them in, plus the signature if the form required one, and Download PDF makes a PDF record of the submission.
Create, link or dismiss
- Create new patient asks for a first and last name, saves the email from the form onto the new patient, and takes you to their page.
- Link to existing patient opens a patient search, for a returning patient or a second submission from the same person.
- Dismiss asks you to confirm. The submission is not linked to anyone, but it stays under the Dismissed tab.
Creating or linking also adds a short note to the patient's notes recording the form name and date. If the form has an email answer, a Create calendar invite preview box is ticked, which prepares a portal invite for that address. Untick it if you don't want one. The answers stay on the patient's Forms panel afterward.
When Goji already has a guess
A form sent from a patient's own booking confirmation knows its appointment, so the card shows their name and offers Accept for that patient instead of the choices above. It still waits as Pending until you accept it. If a family member filled it in, Link to a different patient is beside it.
For any other intake, Goji looks for an existing patient whose email matches the form's first email answer exactly, ignoring capitals. If it finds one, the primary button becomes Link to that patient's name, with a line saying it was matched by email and a way out: Create new patient instead, or Link to a different patient. It's only ever a suggestion, and you still press the button.
Contact messages
A contact form has no patient to match. Its card offers Reply, which opens an email to the sender, and Mark handled. Create patient and Link to existing patient remain as smaller buttons for someone who goes on to book. Once the inbox holds a contact message, All types, Patient intake and Contact filters appear above the list.
