Two independent switches, not one all-or-nothing setting: public booking lets anyone with the link pick a service and time with no account or password, while patient-portal booking opens the same calendar only to patients you’ve already invited. Run either, both, or neither — a lot of practices start invite-only and open the public link later once they know what to expect. Every new online booking lands in a staff queue rather than silently appearing on your calendar, so nothing books itself without you seeing it.

One reservation system behind every booking channel, so nothing gets double-booked.
A time slot can be booked from more than one place — your own calendar, the public booking link, or the patient portal — and all three run through the same underlying reservation system rather than three separate booking paths bolted together. Picking a time online holds it for a few minutes while that person finishes; the hold releases automatically if they never confirm, and locks in the instant they do, before anyone else booking at the same moment can take it.
