Submission PDFs

Download any form submission as a PDF with your practice's name and logo, the answers, the agreement wording and the signature details.

Tactile editorial artwork for Forms Pdf

A new patient's insurer, a referring physician or your own paper file asks for the intake she filled in last week. You don't want to copy answers out of a screen. You want one file with your practice's name at the top that reads the way she filled it in.

On the inbox card, and again on the patient's Forms panel.

Where to find it

Every card in the submissions inbox has a Download PDF button, whatever its status, including Dismissed. Once a submission is linked to a patient, the same button sits inside each entry of the Forms panel on their page. While the file is built it reads Preparing PDF…, then your browser saves it. Anyone signed in to your practice can use it; there is no separate permission.

The file is named after the form and the submission date, such as new-patient-intake-2026-10-03.pdf. Long form names are shortened.

What is on the page

The header carries your practice name and logo, with the form name as the title and Form submission beneath it. A small table gives the Date submitted, Time submitted and Status. The logo and accent color are the ones in Settings → Billing, shared with your invoices (branded documents). The accent tints the title. If the submission is linked to a patient, their name appears as Patient.

Under Responses, each question is listed with the patient's answer. Yes/no answers read Yes or No, several choices are joined with commas, and dates follow your practice's date format. A question left blank shows a dash, so you can see it was asked. Long-text answers keep their line breaks, with the formatting markers removed, and a question stays together with its answer across a page break.

If the form had terms blocks, an Agreements section prints each heading and its full text, with Agreed or Not agreed below. A signed submission ends with a Signature section: the attestation as the patient saw it, the drawn signature, Signed by the typed name, the date and time, the IP address and the browser. Every page ends with Generated by Goji.

Questions and answers first, then agreements, then the signature.

It reads the same later

A submission keeps its own copy of the questions as they were asked, and a signature keeps the wording it was given against. Edit or delete a question afterward and an earlier PDF still lists what the patient actually answered.

Good to know

  • The PDF is made fresh on each click and Goji doesn't store the file. Once you save it, it is a copy outside Goji, so handle it like any other patient record.
  • Each download is recorded in your access log with who and when, the same as opening a chart.
  • It carries what the patient wrote, not the patient's other details. Their name appears only once you've linked the submission.
  • If the file can't be built, the button shows a message asking you to check the logo under Settings → Billing, and nothing is saved.

Try it in your own practice.
