A new patient should arrive with the paperwork done, not a clipboard. The form builder is where you decide what you ask, in which order and in whose words, and what the patient sees is a quiet, full-screen flow on their phone.
Build it question by question
Open Forms in the sidebar, switch to the Forms tab and choose New form. Each question has a Question field for the wording and an Answer menu for its type. The nine types are the same ones you use for patient custom fields, so an answer maps cleanly onto a patient record later:
- Free text (short) and Free text (paragraph)
- Yes / No
- Single select and Multiple select, each with a list of options you can add, reorder and remove
- Date
- Percentage and Number
Move questions up and down with the arrows beside them, and remove one from its ⋮ menu. A Number question can take a Min, a Max and a Step; a Percentage takes a Step. The Display control sets a question to Narrow, Half or Full width, which matters when questions share a screen, as in grouped questions.
There is no Save button. Every change is saved as you make it and reported in the save indicator, and if a save fails the builder says Couldn't save with the reason instead of failing quietly.
Required answers, hints and defaults
Tick Required and the patient can't move past that question blank; the question gets an asterisk and the message Please fill this in. An email answer has to look like an email address, and a short-text question with phone in its wording has to contain at least seven digits. The same checks run again when the form is submitted, so they don't depend on the patient's browser.
Each question has an Advanced drawer for the finer points:
- Hint: short helper text that always shows under the question.
- Tooltip: longer guidance, shown from an info icon.
- Answer placeholder: example text that disappears as the patient types, for text, paragraph, email and number questions. It is never submitted as an answer.
- Default answer: a real pre-filled value the patient can change, such as an outline in a paragraph question.
Name, welcome and ending
On the form's Edit form page, the Internal form name is the label your staff see. The Welcome headline, Welcome description and Browser & share title are what the patient sees, and any you leave blank fall back to the internal name. Under Ending you can write the thank-you screen; see custom thank-you screens. The Type setting chooses between a patient intake and a contact form.
You don't have to start blank. Add name fields adds First name and Last name on one screen, Add terms & conditions block adds a section the patient must scroll through, and a free template gives you a finished form to edit.
What the patient does
The patient taps the form's public link, reads the welcome screen and chooses Start. Each screen shows one question, with a progress bar and, where it makes sense, the hint Press Enter to continue. They can go back at any point, and their answers are kept on the device as they go (progress is saved). If they finish a form and open the same link again on that device, Goji says they've already submitted it rather than taking a duplicate. A required signature, if you add one, is its own last step (signatures), and a short form can show everything on one page (one-page forms).
