A patient's new orthopedist asks for your notes from the spring. You don't want to screenshot a screen or retype a chart. You want a file that looks like it came from your practice.
Where to find it
Open a charted visit and its details page shows Edit, Superbill, Download PDF and Delete beside the status. Click Download PDF; the button reads Preparing… while the file is built, then your browser saves it. The file is made fresh each time you click, so it always reflects the chart as it stands, and it is named after the patient's last name and the visit date, such as session-Rivera-2026-10-03.pdf. Anyone on your team with access to the practice can download it; there is no separate permission. It works on a draft as well as a finished chart.
What is on the page
The header carries your practice name with its logo, address, phone and Tax ID, under the document title Session record and the name of the service. The accent color tints the title. Logo and color come from the same place as your invoices and receipts (Settings → Billing, see branded documents), and the address and phone follow the same location overrides the superbill uses.
Under it, a small table gives the patient name, date and time of the visit, provider, location and status. Then comes a Chart section with one block per field. Fields with nothing in them are left out, except Pulse, Tongue, Pattern, Points and Notes, which are shown as a dash when empty so you can see they were looked at. If the chart has no content at all, the page says so rather than printing blanks.
Your chart template shapes it. A section the template doesn't use is left off, and a section you renamed carries your name. Custom fields print first, then pulse (with left and right by position, if you chart it that way), tongue, pattern, technique, points and the note. Points show their side, such as ST36 (L). Repeater fields print one line per entry.
Notes without the markers
Notes and long-text fields are typed with light formatting. In the PDF the markers are removed, so you read the words, not the asterisks or pound signs. One thing to know: the text is printed as a single run, so line breaks and lists in a long note are joined into one paragraph.
Signed and unsigned
The Status row reads Draft, Complete or Signed. A signed chart adds a line at the end, Signed by, followed by the name and the date and time. An unsigned one has no signature line. Every page ends with a footer of Generated by Goji and Page 1 of 2.
What it leaves out
It doesn't include diagnosis codes, charges or CPT lines, or the save history. It also doesn't print the patient's date of birth or contact details. For anything billing-related, use the superbill. If the file can't be built, the details page shows Couldn't generate PDF with the reason, and nothing is saved.
